Our LTC pathway is made up of several stages, all designed to ensure effective and targeted care. The method is based on personalisation and constant monitoring, so that it can adapt to every need and promote the overall well-being of residents.
Initial assessment and planning
A detailed medical history allows us to identify specific needs and draw up a personalised rehabilitation plan, to be carried out both in the residence and at home.
Carrying out the care plan
Targeted interventions – physiotherapy, occupational therapy and psychological support – monitored and adjusted according to the progress made.
Ongoing monitoring and follow-up
Advanced technology allows progress to be monitored daily, with prompt interventions and constant dialogue with residents and families.
